Divider sheets are 110-lb. stock printed with guidelines for fast and accurate placement of medical chart divider index tabs. Sheets are conveniently ruled for making notes and are 7-hole punched to be used with fasteners in folders or 3-ring binders.

Each eight-tab pre-collated set includes seven of the most popular medical index titles used today, plus one writable tab. Made of sturdy 90 lb. stock. Tabs are reinforced with tear-proof colored Mylar. Tab titles include: Correspondence (orange), History & Physical (blue), Hospital (red), Insurance (brown), Lab/X-Ray (green), Miscellaneous (pink), Progress Notes (yellow) and white writable blank tab.

Confidentially sign in your patients. Forms limit access to personal information as required by HIPAA. Patient signs in and waits to be called by name or number. Office staff removes numbered label strip with patient name and adheres it to patient chart to help improve record keeping. Once label strips have been removed, the patient information is now protected behind special backing paper. The sign-in form also provides confidential daily patient log. Forms numbered 1 - 23.
